The reporting options below are available for communicating questions or concerns to Dose Health's Compliance Officer for further investigation.

When making a report, please provide as much detail as possible. You may choose to remain anonymous, but doing so makes it impossible to follow up on your question or concern.

Please note, this is not an emergency service. If you need immediate medical care or are in immediate danger, please call 911.

Dose Health prohibits retaliation against anyone who reports a concern in good faith or cooperates with a review.
